Volunteer Award Form

Who is eligible for an Award?

• Residents of the George Town local government area.
• People who volunteer (must not be paid for activities and efforts on behalf of the community, with the exception of reimbursement of out-of-pocket expenses incurred). 
• Activities must be undertaken through the volunteer’s own free choice and without coercion, for a not-for-profit organisation.

The application should address the following:

• Brief role of the volunteer’s work.
• Contribution made to the community through volunteer work by the nominee.
• Nominee’s volunteering background, relevant information, attributes and achievement.s

An individual volunteer may be nominated and seconded by an organisation or by other individuals, on the official nomination form.

A nominee, who is not successful in one year, can be nominated again in any subsequent year.

All nominees, family and friends are invited to attend a function to be held on Wednesday, 21st of May 2025 at 5:30pm at the George Town Memorial Hall.

How to compile your nomination:

• Obtain the nominee’s acceptance.
• Describe the voluntary achievement, background and the reasons for the nomination.
• Enclose copies of any newspaper or magazine articles about the nominee relevant to volunteering (please do not include originals).
• Include any other relevant information (please attach additional pages if required).
